Case Management
Northside’s case managers provide a wide range of supports to frail older people, people with a disability and their carers. Some of the ways in which case managers can support people include:
- Linking people to other NCS services such as transport, the shopping bus, social groups or 1-1 social support;
- Referrals to other community providers for support with such things as home modifications, gardening or counseling;
- Assisting with the coordination of multiple services;
- Advocacy and advice for additional support when dealing with service providers.
In addition, Northside has a cultural case manager who can provide the same range of services for people from culturally and linguistically diverse backgrounds.
