Do you want to keep living independently in your own home but need a little extra support?

The Support at Home Program, starting 1 November 2025, is the Australian Government’s new way of funding in-home aged care. It replaces the old Home Care Packages and Short-Term Restorative Care programs and brings together a simpler, more flexible system that gives you more control and choice over your care.

At Northside Community Service, we have been supporting older adults for over 50 years. Our goal remains the same – to help you live safely, confidently, and joyfully in your own home for as long as you choose.

What the Support at Home program offers

Under the new system, you can receive tailored, government assisted support to help you stay well, connected, and independent.

Services may include:

  • Domestic assistance such as cleaning, laundry, and meal preparation.
  • Personal care such as help with showering, dressing, or grooming.
  • Transport to appointments, shopping, and community activities.
  • Social support and companionship.
  • Home maintenance, equipment, and minor modifications.
  • Short-term or restorative care after illness or hospitalisation.

Every client has a dedicated Care Coordinator who will work with you to create a personalised care plan, regularly reviewed as your goals or needs change.

Your co-contributions

Most people receiving aged-care services are asked to pay a contribution toward the cost of their care. Your exact amount is assessed and advised by Services Australia before your services begin.

A dedicated care coordinator who will work with you to develop a comprehensive care plan, including regularly reviewing your budget and plan as your goals and care needs change.

Northside will always provide clear information about your care plan and budget.

Engaging Northside to coordinate and deliver your Support at Home services:

Our friendly team is here to help you understand your options and start living the life you want – in the home you love.

If you are new to aged care, you can start your journey in a few simple steps:

  1. Call My Aged Care on 1800 200 422 or visit myagedcare.gov.au to register and complete an assessment.
  2. Once you have a referral, nominate Northside Community Service as your preferred provider.
  3. We will guide you through your service agreement and care plan, explain your funding and co-contributions, and coordinate your supports.

We will work with you if you require assistance through the process.

If you are currently receiving aged care services and want to switch to Northside:

  1. Contact your current package provider to let them know that you no longer require services from them. You will need to advise and agree with your current provider the date when your services will cease.
  2. Once we have your My Aged Care referral code and the date you have agreed to end services with your current provider, we will commence the transition to Northside. One of our coordinators will then visit you at home to develop your care plan, which of course, will be guided by you based on your needs and budget.
